Registration & Enrollment

There are several forms that MUST be completed before your child may attend Bundles of Joy. The enrollment agreement must be read and signed by the parent. A nonrefundable registration fee and a deposit will be applied.

A health inventory, immunization certification, emergency card, child release, and a family information form must be completed and returned. The health inventory and immunization forms must be filled out by a physician.

Every child enrolled has a two week trial period in which Bundles of Joy reserves the right to un-enroll that child if we discover that there are special needs or behavioral issues that cannot best be met by our staff. On the rare occasion that this would happen, we would make every effort to help you find care that would accommodate you and your child.
